 Before arriving at Grace Home, our staff asks that you be "ready to go to any length" to turn your life around and begin a life of sobriety. You must also agree to changing your associations (Playmates & Playground).

 

Once you arrive at Grace Home it is expected that you are truly ready to find a better way of life and the best that life has to offer. Unless you are willing to change, everything in your life will stay the same. To make life changes will not be easy... In fact, you will have time of struggles and tests during your stay at Grace Home. If you remain honest and open during these times, struggles will turn into "life lessons". Being open to these "life lessons" will encourage you to renew your commitment to attain your goals.

 

We don't promise that your time at Grace home will be easy and there are requirements that you MUST agree to while you are residing at Grace Home:

  • House rules and curfews MUST be adhered to.
  • Priorities MUST be maintained. Grace Home and sobriety/ abstinence is your TOP priority. You are in recovery to improve your quality of life. All classes and appointments MUST be kept. Residents MUST be active in treatment and children (age appropriate) MUST attend school.
  • Each resident MUST accept being an active participant in Grace Home and learn the responsibility that goes along with pulling your weight by adhering to chore schedules, mealtime and bedtime rituals required by the Grace Home program.
  • Residents MUST maintain a positive attitude toward and show a willingness to work with and get along with the other residents of Grace Home and the staff at Grace home. Any negativity expressed to another resident on Gateway Community Services' property or anywhere else can result in immediate dismissal.

* Further information regarding Grace Home's Rules and Regulations will be provided to residents upon their arrival.


Phases:

 

Orientation Week (2 Week timeframe):

  • Visitor contact will be limited to USPS (mail). There will be no phone, internet or face-to-face contact during this phase.
  • Resident will complete a chemical dependency assessment at Gateway Recovery Center.
  • Resident will begin attending treatment at Gateway Recovery Center.
  • Attend a minimum of two (2) 12-Step Meetings weekly. You MUST be accompanied by a Phase II or III resident.
  • Set a financial budget with the Case Manager.
  • Apply to Temporary Assistance for Needy Families (TANF).
  • Apply to Family Connections for child care and child care assistance.
  • Meet with your Case Manager weekly.
  • Other tasks may be assigned by your Case Manager based on your family's individual needs.

Phase I:

  • Restriction to Grace Home with the exception of  12-Step Meetings, treatment at Gateway Recovery Center and mandatory lifeskill appointments.
  • Healthy Relationships Curriculum sessions started.
  • Attain a temporary 12-Step sponsor before the completion of Phase I.
  • Attend a minimum of two (2) 12-Step meeting per week. Must be accompanied by Phase II or III resident.
  • Minimum weekly contact with your 12-Step sponsor.
  • Visitor contact must be pre-approved by the Case Manager. This includes anyone you will have personal contact with including access via telephone, mail and internet.
  • With your Case Manager, you will establish a plan to address your individual life skills needs and begin to implement the steps necessary to reach each goal.
  • Submit a weekly schedule of all activities.
  • Meet with your Case Manager weekly.
  • Participate in all program requirements, life skills assignments, children evening rituals and chores.
  • Other tasks as assigned by your Case Manager.

Phase II:

  • Pre-approved, but limited outings.
  • Employment and Educational goals set.
  • Attend a minimum of two (2) 12-Step meetings per week.
  • A limited number of contacts with approved visitors or guests. All contacts must be pre-approved.
  • Continued implementation of steps necessary to achieve personal action plan goals and objectives.
  • Recognizable improvement and stability.
  • Submit a weekly schedule of all activities.
  • Meet with the Case Manager weekly.
  • Participate in all program requirements, life skills assignments, children evening rituals and chores.
  • Other tasks as assigned by your Case Manager.

Phase III:

  • An increased number of pre-approved outings.
  • Attend a minimum of two (2) 12-Step Meetings per week.
  • A limited number of guests or visitors. All visitors must be pre-approved.
  • Overnight stays away from Grace Home may be permitted with at least a two week advance notice.
  • Achievement of personal goals that includes senior resident status.
  • Preparation for program completion to include attaining housing.
  • Submit a schedule of weekly activities.
  • Meet with your Case Manager weekly.
  • Participate in all program requirements, life skills assignements, children evening rituals and chores.
  • Other tasks as assigned by your Case Manager.

Phase Reviews:

 

Phases are in place to help you structure your living arrangements. You are responsible to meet with your Case Manager one time per week. Phase Reviews will be conducted by the Case Manager on a regular basis as needed. The Phase Reviews have points assigned to the tasks in each phase, you must score at least a 91% to advance to the next phase. Residents reaching Phase III, have the option of staying at Grace home until an appropriate amount of savings has been accumulated.

 

Progress reports are done periodically and coordinate your treatment progress as well as your commitments in Grace Home. The Case Manager has the right to evaluate your performance in any phase at any time. If it is determined that you are not complying with the standards of a given phase, Grace Home staff can revoke the privileges of that phase.
